Athletic Registration (Fall Sports)
Does your child plan to play a fall sport at Henley High? If so, you must come in to the Henley High Main Office on one of the following dates and you must have all items on the checklist below turned in / completed before the first day of practice on Monday, August 15.
Wednesday, August 10 11:30 a.m. to 1:30 p.m. and 3:00 p.m. to 6:30 p.m.
Thursday, August 11 10:30 a.m. to 1:30 p.m. and 3:00 p.m. to 6:30 p.m.
Friday, August 12 OFFICE CLOSED
Monday, August 15 7:30 a.m. to 11:30 a.m. and 1:00 p.m. to 4:00 p.m.

IF YOU ARE UNABLE TO REGISTER ON ONE OF THE ABOVE LISTED DATES, YOUR STUDENT WILL NOT BE ELIGIBLE TO PARTICIPATE IN THE FIRST WEEK OF FALL PRACTICE. OUR OFFICE WILL BE CLOSED FROM AUGUST 16 – 22. THE OFFICE WILL REOPEN AUGUST 23 AT 8:00 A.M.
Requirements for Athletic Eligibility “Blue Card”:
_____ Completed School Registration Form
_____ Signed Athletic Participation Form (must be completed yearly)
_____ Completed Physical Form We recommend physicals be completed between June 15 and August 10. OSAA requires a physical following the 8th grade year and prior to participation
in high school. Physicals are valid for two years.
_____ Parent Athletic Transportation Consent Form
_____ Purchase Student Body Card - $20
_____ Athletic Fee - $100
_____ Cleared from school fine list
_____ Grade verification
Once these items are turned in / completed you will be issued a “Blue Card” to give to your coach.
All athletes MUST be cleared through the Henley High School office and
MUST possess a “Blue Card” prior to the first practice on Monday, August 15.
**Athletes WILL NOT be allowed to practice without a Blue Card**
Impact Concussion Testing is also required to participate.
Testing will be completed with coaches on the first day of practice.
(Forms can be found on the Henley High School website. They will also be available at registration in August.)
As we reach the end of yet another school year, I would like to take the opportunity to thank you all for your help, support and interest this year. We have had another very busy and successful year in school and you, as partners in the education of your children, deserve much of the credit for that.
Please read the following letter as it contains important information for next year.
Tuesday, August 23 – Friday, August 26 8:00 a.m. to 3:00 p.m.
Monday, August 29 – Friday, September 2 8:00 a.m. to 3:00 p.m.
Location: Henley High School Main Office
Stop by the main office during these times to: Update registration forms, pre-pay lunch accounts and pay student fees, such as student body card, athletic & activity fees, hall & P.E. locks, P.E. uniforms, parking permit, yearbook, and fees for classes such as science, shop, art, ROTC, choir & band, and complete Activity Participation Forms. Returning students should bring their locks to trade-in – Hall locks are black, P.E. locks are green.
First Day of School
9th Grade Tuesday, September 6
10th through 12th Grades Wednesday, September 7
Picture Day – September 16
Lifetouch School pictures will be taken on Friday, September 16. Watch your mail or email in August for our picture day id number so you and your student can be prepared for picture day.
